Job Description
Locum 4 Nurses is looking for a Locum Practice Nurse to run long‑term condition clinics for a GP practice in Wolverhampton.
Role summary
This post is designed for a Practice Nurse who enjoys structured long‑term condition work and supporting QOF‑aligned outcomes.
Main duties in a GP practice
- Deliver chronic disease management clinics (e.g., diabetes, asthma/COPD, hypertension) using practice templates.
- Undertake structured reviews, adjust care plans, and escalate to GP or specialist services when required.
- Perform clinical observations and relevant investigations within competence (e.g., spirometry where trained).
- Contribute to QOF outcomes through accurate coding and proactive follow‑up of outstanding actions.
- Provide patient education on self‑management, inhaler technique and lifestyle changes.
Working pattern
4 days per week (consistent block booking).
What we’re looking for
- Registered Nurse with strong primary care LTC experience.
- Confident running independent clinics and working to protocols/PGDs.
- Excellent written records and safe clinical judgement.
- Reliable and able to maintain continuity across multiple weekly sessions.
Systems & ways of working
- Experience with LTC templates, recall systems and results workflows.
- Good understanding of escalation pathways and safeguarding in primary care.
- Team‑orientated approach with clear handovers.
