Job Description

Locum 4 Nurses is seeking an Advanced Clinical Practitioner to assess undifferentiated patients as a first‑contact clinician for a GP practice in Chorley.

Role summary

This role is suitable for an ACP who can operate as a first‑contact clinician in primary care, adding safe capacity and leadership to the GP practice team.

Main duties in a GP practice

  • Provide autonomous assessment, diagnosis and management of undiagnosed/undifferentiated presentations.
  • Deliver same‑day capacity, including urgent triage and rapid decision‑making with appropriate escalation.
  • Request and interpret investigations, formulate differential diagnoses and document robust clinical reasoning.
  • Work across the multidisciplinary team, coordinating with GPs, pharmacists, practice nurses and PCN services.
  • Support clinical leadership through advice, mentoring and maintaining high practice standards.

Working pattern

Flexible sessions (2–3 days per week, mix of morning/afternoon).

What we’re looking for

  • ACP (nursing or AHP background) working at an advanced level with evidence of competence in primary care.
  • Strong critical thinking, risk management and escalation decisions.
  • Excellent communication and ability to manage complexity under time pressure.
  • Commitment to reflective practice, audit and continuous improvement.

Systems & ways of working

  • Experience in urgent care/primary care pathways and referral management.
  • Comfortable with clinical supervision structures and governance.
  • EMIS Web or SystmOne experience (desirable).
