Job Description

Locum 4 Nurses is looking for an Advanced Nurse Practitioner to run chronic disease and long‑term condition clinics for a GP practice in Workington.

Role summary

This role is designed for an ANP who enjoys continuity and structured clinics, supporting population health and long‑term outcomes.

Main duties in a GP practice

  • Deliver LTC reviews (e.g., asthma/COPD, diabetes, hypertension) and optimise care plans in line with practice templates.
  • Review results, adjust treatment, and arrange monitoring including escalation to GP when needed.
  • Provide health promotion and preventative care, including structured medication reviews where appropriate.
  • Support QOF‑related activity through accurate coding and high‑quality clinical documentation.
  • Work with the practice team to reduce avoidable appointments through proactive care planning.

Working pattern

4 days per week (regular rota for continuity).

What we’re looking for

  • ANP with NMC registration and substantial general practice experience in LTC management.
  • Excellent clinical assessment skills and ability to manage complexity and comorbidity.
  • Confidence using clinical templates, protocols and coding standards.
  • Professional, calm communication style with patients and colleagues.

Systems & ways of working

  • Experience with QOF/LTC templates and outcome‑focused reviews.
  • Confident interpreting investigations and documenting action plans.
  • Team‑first approach and reliable attendance across set sessions.
